IDDS ADULT CASE MANAGEMENT

  

Most individualswith Intellectual Disabilities require assistance with securing medical care, financial support, housing options, developing social support groups, and employment opportunities.  Case Managers provide assistance in obtaining services needed to live successfully in the community.

Case Managers monitor individual success and assure that the needed services are provided and are to the satisfaction of the individual and family.

The Case Manager will work with the individual and family to develop a Person Centered Plan that will build on strengths, needs, and preferences. 

 

 



Services

*  Assessment of the individual strengths, needs, preferences, and desires.

*  Assistance with developing a Service Plan with the consumer's natural support system.

*  Assist in the linking and monitoring of services to ensure the success of established goals. 

*  Assess the need for placement on the Medicaid Waiver Waiting List.

*  Coordinate discharge planning from state facilities.

*  Provide Crisis Intervention Services to ensure the health and safety of individuals and care providers.

 



Eligibility Criteria

 * Individuals age 18 and older

* Resident of Hampton or Newport News area

* Psychological Assessment reflecting the current intellectual and adaptive functioning
