Welcome to Adult Male Mental Health and Dual Diagnosis Case Management Services!

Case Management provides assistance to individuals with severe mental disabilities to obtain the services they need to live productively in the community.  Because services can be fragmented in many areas, case managers provide a critical function to monitor a person's needs and assure that appropriate agencies get involved.

Case Managers can be nurses, social workers, or mental health therapists.

The Case Manager will work with you and/or your loved one to develop an individualized plan that will build on your strengths and preferences to have control of your disability.

Preferences in Admission to Substance Abuse Treatment Programs

Hampton-Newport News Community Services Board must give preference in admission to pregnant women who seek or are referred for and would benefit from substance abuse treatment. The agency must give admission preference to individuals in the following order:
·       Pregnant injecting drug users,
·       Other pregnant substance abusers,
·       Other injecting drug users, and
·       All other individuals.
 
Following completion of a comprehensive substance abuse assessment, individuals may be enrolled in treatment services.
 
South-Eastern Family Project- A collaborative effort to provide comprehensive, residential and day substance abuse treatment for pregnant women. Mothers and newborns can remain together in a healthy, safe, and stable environment. Mothers receive clinical help with their addiction and emotional problems, while learning daily living and parenting skills that will foster healthy child development.

Hampton Roads Clinic- The Opioid Treatment Program at Hampton Roads Clinic offers a unique and specialized program for adults with a long history of dependence on opiates such as prescription pain medications, heroin, and morphine. The program offers a full range of services to help individuals achieve recovery from opiate dependency and improve their quality of life.

Project LINK- Project LINK is a case management program aimed at assisting women and their children who have been affected by substance abuse. Through the use of innovative linkages, a full continuum of care is provided that integrates prevention, early intervention, and substance abuse treatment with health care and other support services.

Partners in Recovery-Services are designed for individuals who are encountering serious psychiatric problems along with their abuse or dependence on alcohol or drugs. Members of the treatment team integrate psychotherapy, medication management, relapse prevention, and life-skills training with 12-step recovery, crisis intervention and social developmental orientations to support participants in achieving their goals.

Free Services will include:
 
1.      HIV education
2.      HIV testing

No appointment is necessary. This clinic is open to all regardless of diagnosis, disability, etc.
